How the Referral Program Works:

• Call, email us, or complete this form and return it to us, with the names and contact information of the businesses you are referring.
• Please call them to let them know to expect a call from us, and then we will call each of them to schedule an appointment.
• We will send you a personal thank you check in the amount of $25 for each referral that makes an appointment to meet with us.
• If your referral becomes a client, we will send you another check, this one for $500!

How the contest works:

• Refer your friends and colleagues.
• Each friend who becomes a client will bring you one step closer to winning.
• To be eligible for the contest, two or more of your referrals must become clients.
• If you refer more new clients than anyone else by June 30, 2012, you will be the winner of an iPad2 or an Amex Gift Card of equal value!*
• The winner’s name will be published on our website.
